RETURN POLICY

Describe the process carried out by DR. CARLOS PEREIRA , for the request for the return of monies given by clients.

  1. SCOPE:

This document applies to the decisions and actions taken to accept the return of monies given as payment for the services offered by the DR. CARLOS PEREIRA

  1. DEFINITIONS:
  • Policy: Guidelines given by the DR Management. CARLOS PEREIRA for making decisions according to each situation.
  • Cancellation: is the cancellation in a timely manner of the performance of a service previously contracted with the DR. CARLOS PEREIRA.
  • Return: Situation in which the client or any user of the DR. CARLOS PEREIRA, requests the return of the money delivered for the service before it was consumed.
  • No show : Appointments not attended without prior cancellation.

  1. GENERAL POLICIES FOR REQUEST FOR CANCELLATION OF CONTRACTED SERVICE AND RETURN OF MONEY.

To cancel the contracted service and refund the money, the client must download the MONEY RETURN REQUEST FORMAT file, which can be found on our website https://drcarlospereira.com/politicas-de-devolucion/ .

The above must be completed and delivered in person at the company's facilities or via email to email: admin@drcarlospereira, relating the reasons why you are requesting the return and cancellation, the details of the account in which it will be done. the return, providing a copy of all payment supports and other documents that demonstrate conditions, reasons or reasons that support the cancellation and return request.

CAUSES TO REQUEST A RETURN

  • When purchasing a service offered by DR. CARLOS PEREIRA , and when making full payment or payment of the value of the service, the client will have a maximum period of five (05) business days from the date on which the payment was made, to exercise their right of withdrawal, in accordance with the provisions of article 47 of the consumer statute, without the DR. CARLOS PEREIRA any withholding of the delivered value is made. PARAGRAPH: When the provision of the purchased service must be carried out within 5 business days from the date the total or partial payment was made, the Right of Withdrawal will not apply.
  • When the refund request is submitted within 48 hours of the date on which the service is scheduled to be provided, the company will refund the money delivered to date, applying a 20% withholding.
  • If the refund request is made within 48 hours prior to the date on which the service is scheduled to be provided, the company will not refund the money delivered to date.

 

  1. PERIOD FOR RETURN OF THE MONEY

For all the cases stated above , DR. CARLOS PEREIRA will have a maximum period of 30 business days from receipt of the request to refund the money. If the monies delivered have been received in dollars, the refund will be made taking into account the TRM that was on the date the money was delivered.

  1. SPECIAL POLICIES FOR REQUEST FOR CANCELLATION OF CONTRACTED SERVICE AND RETURN OF MONEY.
  • Money given for consultations will not be refundable .
  • The money received for the acquisition of promotional services and savings plans will not be refundable.
  • In the event of the death of a family member who is up to the fourth degree of consanguinity or first degree of affinity, a refund of 80% of the total or partial amount paid to carry out the contracted procedure until the date of request will be effective, applying the withholding of the 20%, which can be used within the next six (06) months from the date on which the refund becomes effective. The patient may use that 20% to perform any procedure with the DR. CARLOS PEREIRA , if no procedure is carried out within the previously stipulated term, the retained value cannot be returned or used in any treatment and/or procedure.
  • In the event that the patient is deprived of his liberty and notifies in accordance with the procedure stipulated here, 48 business hours prior to the scheduling of his surgery, a refund of 80% of the total or partial value paid for the contracted procedure will be effective. The remaining 20% ​​will be retained for a period of one (01) year from the date on which the return becomes effective. If no procedure is performed within the previously stipulated period, the retained value cannot be returned later nor can it be used in another treatment and/or procedure.
  • For canceled and/or rescheduled appointments/surgeries, an extra charge of 10% of the total service value will be generated. Which will be deducted from the advance payment given.
  • For appointments/surgeries canceled and/or rescheduled with less than 24 business hours to the date of the procedure, an extra charge of 20% of the value of the service will be generated, a value that will be deducted from the advance payment given.
  • For unattended / no-show appointments, the entire amount paid will be lost.
  • It is clarified that, for the purposes of determining the time of money refund requests, the initial scheduled date will be taken into account and not the new rescheduled date.
  • For all the cases stated above , DR. CARLOS PEREIRA will have a maximum period of 30 business days from receipt of the request to refund the money.
  • In other specific cases, refund requests will be considered by the legal department and management.
